Health Educator – Community Outreach Must be Bilingual - Fluent in Spanish Minimum
Qualifications: High School Diploma. Experience and/or training in clinical settings preferred. Administrative support experience required. Satisfactory professional references. Experience: 1-3 years of clinical/professional experience Must be bilingual. Demonstrating excellent oral and written skills in the language identified by the Health Center as being a language-of-need. Demonstrated technical experience with Microsoft Word, Excel (or similar database) Chrome, Internet Explorer, and cell phone applications Able to function in a flexible work schedule to include evenings and weekends as required by the job and as assigned by the Director Valid NC Driver License Acceptable Driving Record Duties CPR certified or certification attained within the first 6 months of employment.
Provide basic education, screening, and support and/or facilitate referrals to other health center or community-based resources. Basic patient education will include but is not limited to: HIV, STD, and other chronic diseases such as Asthma, Diabetes, Hypertension Healthy weight management Depression Domestic Violence Tobacco cessation Pediatric/Parenting Education Health system education and advocacy to assist patients in navigating the healthcare system
Provide assistance and support for transportation Participate in population based projects that enhance retention in care, self-management, treatment, adherence and compliance with accepted health care standards Participate with community, city-wide or statewide resources and/or professional development with other organizations such as, but not limited to NCFW, NC DHHS, NCCHCA Participate in assigned community based activities as assigned, including outreach to potential new patients and connecting them to care and services, and will assist with the set up and breakdown of event props. (Table, chair, displays) as applicable. Responsible for accurately completing mileage logs, patient assessments, and other record keeping. Interprets and translates/speaks Spanish regularly for clinic operations/events.
Regularly, no less than every other day, completes warm hand-offs i.e. turns in health assessment for billing, scanning, and entry into the EHR. Initiates the patient registration process and correctly completes associated forms, and gathers required documents. Explains all appropriate resources for assistance including 340B, PAP, ICOR, DSS, LCTS ridership, DSS Medicaid transportation vouchers, WIC and appointment-based care (PCMH) as opposed to the acute walk-in care; Educates patients to comply with all center requirements for care. Follows protocols and utilizes sound judgements to appropriately respond to requests for services.
This may include facilitating the scheduling, documenting outcomes, and notifying the patient of the pending appointment. Regularly communicates with patients, staff, and community organizations. Provides blood pressure checks, blood sugar checks, and performs other simple clinical measurements. Works without direct supervision; adheres to company policies and notifies the Director immediately of barriers to implementing the job as outlined.
Communicates and follows instruction from Outreach Care Coordinator and Community Development Director regularly regarding Outreach to Migrant/Seasonal farmworkers, families and other underserved populations in the community.
