LENOIR COUNTY GOVERNMENT HEALTH DEPARTMENT SOCIAL WORK SUPERVISOR II

SALARY RANGE: $36,222 - $55,116

DESCRIPTION OF WORK:
• To serve as Supervisor for CMARC and CMHRP for Nurses and Social Workers.
• To be proficient in the use of Patagonia, Virtual Health and NCIR.
• To ensure home visits are made for the purpose of case management in a timely manner.
• To evaluate the performance of and effectiveness of staff.
• To serve as a resource person for Case Managers and staff with questions about CMARC/CMHRP.
• To provide Nursing Director with data that present the outcomes of LCHD Case Mgt. Programs.
• To contact community partners on a quarterly basis to make sure the referral process is understood and is happening to ensure healthy outcomes for families of Lenoir County.
• To market these programs with community partners to ensure understanding and proper referral and use of the program.

KNOWLEDGE, SKILLS & ABILITIES AND TRAINING & EXPERIENCE REQUIREMENTS:

• Considerable knowledge of and skill in the application of theory, practices, principles, and techniques employed in the field of public health and related programs.
• Considerable knowledge of and ability to apply the principles and practices of public health.
• General knowledge of current social and economic problems relating to public health.
• Considerable knowledge of available resources and organizations.
• Ability to deal tactfully with others and to exercise good judgement in appraising situations and making decisions.
• Ability to secure the cooperation of others, to elicit needed information and to maintain effective working relationships.
• Ability to record accurately services rendered and to interpret and explain records, reports, and medical instructions.
• Ability to plan, coordinate, and supervise the work of others.
• Ability to communicate, verbally & written, with staff, community agencies, & general population
• Considerable computer skills
• Flexible & open to change to meet needs of the community

MINIMUM TRAINING AND EXPERIENCE:
Master’s degree in social work from an appropriately accredited institution and one year of directly related experience; or a bachelor’s degree in social work from an appropriately accredited institution and two years of directly related experience; or a master’s degree in a human service field from an appropriately accredited institution and two years of directly related experience; or a bachelor’s degree in a human services field from an appropriately accredited institution and three years of directly related experience; or a bachelor’s degree from an appropriately accredited institution and four years of directly related experience; or an equivalent combination of education and experience.
