Help wanted: Lenoir County Health Department - Social Worker II

Entry Salary: $37,444

The Social Worker II is responsible for an assigned patient case load of county residents that varies. Each patient assignment is based upon a referral from a medical provider or other agencies providing services to children 0-5 years who reside in Lenoir County. The social worker is responsible for:

- assisting with obtaining community resources to ensure positive health outcomes

- to teach clients and their families the proper use of medical services

- follow-up with any risk factors that may be detrimental to positive health outcome by using the Life Skills Progression, SWYC, and Community Needs Assessment; developing and implementing a plan of care to ensure healthy outcomes; and providing data information to the North Carolina Division of Medical Assistance's data base – Virtual Health (VH)

Required Minimum Training: Bachelor’s degree in social work (BSW, BA in SW, or BS in SW) or master’s degree in social work (MSW, MA in SW, or MS in SW) from a Council on Social Work Education (CWSE) accredited social work degree program.

License or Certification Required by Statute or Regulation: Current valid N.C. driver's license
