Observations: 

Employee not wearing hair net.

Soda machine leaking water onto floor by drive through. Suspected walk in cooler no functioning properly 

Hand sink by 3-comp sink with steady drip. Leak at cold water faucet handle at 3-comp sink Heavy leak at spray nozzle of 2-compartment sink.

Dumpster lids open. Drain plug missing from dumpster.

Observations: 

Bottoms and shelves in coolers and counters have crumbs, spilled items from moving in and out, etc. Build up on gaskets of cooler doors. Establishment shall clean and maintain.

Leak under 3 comp sink in back area of kitchen, where water is pooling on floor. Establishment shall repair/replace to ensure equipment is working correctly.

Dumpster open upon inspection.

Mop drying upside down.

Observations: 

Neither of the 2 stem thermometers are correct. Staff was instructed on how to calibrate these.

The freezer still has ponding, frozen water in the bottom. Maintain all equipment in good repair. Rubber spatulas are in disrepair. Staff was asked to discard them.

Provide non-reverting measuring device for the dish machine. 

Many of equipment surfaces observed to be soiled.

Better cleaning needed at the wall around the stove area.

Observations: 

Spray bottle all purpose cleaner w/bleach stored on same shelf as sauces in cabinetry under Pepsi machine.

Multiple flies present in facility. Back door weather striping missing/in disrepair; daylight observed under door where weather strip is missing. 

Box of new utensils (mostly looked like food scoop for steam well) sitting on top of black trash can. Utensils were still individual wrapped with manufacture packaging and stored inside shipping box.

Hose used to spray down grease trap under fryers was seen stored next to 3-compartment sink.

Plastic lid to Cambro stored in clean utensil/equipment rack broken.

Medium size white cutting board in utensil/equipment rack stained with heavy scoring.

Black food carts and metal food carts, hot holding cabinets, outside of fryers, walk in refrigerator shelving, steamer, steam tables, Blodgett oven, hoods and other equipment need additional cleaning. 

Date marking stickers still present on dishes stored on the clean utensil/equipment rack. 

Raw meat seen on outside ice machine door. Food debris on the inside.

Hand sink faucets in kitchen area need cleaning.

Dumpster lid open. No plug present. Dumpster area needs cleaning. 

Walk in freezer floor at threshold missing tiles and where tiles are missing concrete is torn up. Excessive amount of grease and dust accumulating on the ceiling tiles, security cameras, ac vents and walls through out the kitchen. 

Walls, floors and ceilings need additional cleaning. Some broken baseboard tiles.

Ventilation hood system is inadequate. Facility is filled with smoke and cannot keep up with the prevention of grease build up collecting on walls, ceilings and equipment.

Observations: 

Marinara sauce cooling in reach in fridge was tightly wrapped in plastic wrap, then a lid was placed on top. Discussed with pic about proper cooling methods.

Observed an oil bottle, ranch squeeze bottle not labeled.

Observed a few boxes of product on the walk in freezer floor under the storage shelves. 

Observed torn gaskets at the two door reach in to the right of the grill flat top.

8176 Hwy 258 S, Pink Hill

Observations: 

No certified food protection manager present. 

Condensation from ceiling AC vents active dripping condensation onto food prep table by 3-comp sink.

Sanitizer in the sani-bucket at the 3-comp sink, and the bucket at the hand sink by the soda dispenser both tested so strong it bleached out the test strips. 

Sausage patty's prepped today cooling in the reach in across from 3-comp sink were in a plastic container with a tight fitting/locking lid. This is not an approved cooling method. 

Multiple sauce bottles/spice shakers not labeled through out kitchen.

Several flies around food service area and seating.

Dead flies were seen in the hand sink below the fly light trap at the drive through window and in the dry storage area.

Wiping cloths shall be stored in a chemical concentration of chlorine (bleach) between 50 and 200ppm.

Hood system is in need of cleaning.

Employee purse stored on top of bulk food storage containers.

Observations: 

Dish machine is not pulling sanitizer.

Microwaves are soiled inside cavities. Microwaves shall be cleaned as often as needed.

Walk-in freezer door propped open because manager said it was cut off earlier today for delivery/inventory, but is not back on.

Two flies present during inspection. 

Insides of refrigeration, detail areas, cracks, crevices, hard to reach areas, etc. are soiled.

Floors need grout. Floors shall be easily cleanable.

Water dripping from ceiling during inspection after a hard rain.

Observations: 

Establishment has violations of priority items, no CFPM, and does not respond correctly to questions regarding public health questions.

No Certified Food Protection Manager present.

Person in charge is not aware of responsibility to inform food employees of reporting certain symptoms, does not demonstrate employees are aware of responsibility to report, and does not provide evidence of training or implementation of a health policy with all required information.

No Vomit and Diarrhea Clean-up Plan provided by establishment.

Food handlers did not wash hands between changing tasks, after walking outside, etc.

Hand wash sink in front kitchen is filled with container and cloths, and is too full for hands to be washed.

No hand drying device provided for front hand wash sink.

Ice build-up inside top of refrigerator. 

Stem-type thermometer provided but does not work. An approved, accurate, working stem-type thermometer shall be provided.

Many flies present. 

Some food handlers do not wear hair restraints. 

Handle of ice scoop in ice bin is in contact with ice. 

Chest freezer in back room is not approved. 

Soiled utensils piled on 2 trashcans, stacked on drainboard for soiled utensils, stacked in wash compartment and rinse compartment of 3-compartment sink. Either batch wash, or provide additional storage space for soiled utensils. There is no additional space in the 3-compartment sink area for the storage of soiled utensils other than the drainboard. Batch washing is strongly suggested. Stacked utensils in sink compartments makes it impossible to wash utensils by approved methods.

No cold water in men's restroom. No water comes out when faucet is turned on.

Lid for dumpster is stuck inside the dumpster.
