Help Wanted: Lenoir County Health Department - Social Worker II

As a member of the Lenoir County Health Department, you can make a difference! The Health Department is seeking candidates for a Social Worker II. The Social Worker II is responsible for an assigned patient case load of county residents that varies. Each patient assignment is based upon a referral from a medical provider or other agencies providing prenatal services to county residents. The social worker is responsible for the care management of the patient’s pregnancy; assisting with obtaining community resources to ensure a healthy pregnancy outcome; ensuring prenatal medical and postpartum services are received; follow-up with any risk factors that may be detrimental to a healthy pregnancy outcome; developing and implementing a plan of care to ensure healthy outcomes; and providing data information to the North Carolina Division of Medical Assistance data base – Virtual Health (VH).

Minimum Requirements:

• Bachelor’s and/or Master’s Degree in Social Work from

an Accredited School

• Possess a valid North Carolina Driver's License

• Pass a criminal background check

• Must be of Good Moral Character
