Observations:
Employees lack knowledge of employee health policy.
Establishment lacks vomit and diarrhea plan.
Hand sink water temp. at 87F.
Cheese is lacking date mark.
Ice machine, dishwashing machine, and XLT conveyor belt soiled.
Several refrigerators, walk-in freezer and fridge, walls, floors, baseboards, hand sinks, behind/under equipment, shelving racks, carts, and storage under prep areas soiled.
Cardboard only refuse open. Garbage area is surrounded by debris.
Visible standing water. Mop sink area also soiled.
Visible mold on ventilation areas.

Dishwasher is not reaching required temperature.
Walk-in freezer floor and walk-in fridge floor, flip-top fridges, hand wash sinks, green storage racks, breading and fryer equipment and wheels, two burners, ovens, and oven doors visibly soiled.
Plug missing from cardboard receptacle.
Floors throughout facility and din areas under equipment are visibly soiled.

Some meat trays cracked and some carts are rusty.

Knife sharpeners and meat scrapers in contamination area.
Observed single use articles in sleeves on the ground in the precessing room and dry storage.
Test strips expired in 2021.
Observed buildup of meat residue on the inside covers of the bandsaw.
Buildup of mildew on gasket to walk in and condenser is dusty in the storage room.
Unisex bathroom did not have a covered trash can.
Bathroom doors were not self closing and opening doors.
Observed holes and rust on dumpster.
No drain plug was observed in dumpster.
Ceiling in stock room not easily drainable.
Observed floor/wall junctions not sealed.
Observed meat/bone splatter on the ceiling above the board-saw.
Dust buildup on the vents in the bathrooms.
